🛡️ מדיניות פרטיות – לב מטפל בספורטאים ושיקום רפואי

(להלן: "האתר"), המופעל על-ידי לב פבלוב, מטפל מוסמך ברפואה משלימה מערבית,
מנהל קליניקות לטיפול ושיקום ספורטאים בתל אביב (להלן: "העסק" או "אנחנו").

העסק מחויב לשמירה על פרטיותך ולשקיפות מלאה באשר לדרך שבה נאסף, נשמר ונעשה שימוש במידע הנאסף באתר.
מדיניות זו נועדה להסביר מהם הנתונים שנאספים, כיצד הם נשמרים, מהן זכויותיך לפי חוק הגנת הפרטיות, התשמ"א–1981, לרבות תיקון 13, ואיך ניתן לפנות בכל שאלה בנושא.

1. איסוף מידע באתר

בעת שימושך באתר, אנו עשויים לאסוף את המידע הבא:

  • פרטים שמסרת בטופסי יצירת קשר: שם, טלפון, טווח שעות רצוי לטיפול, תאריך טיפול מבוקש, ותוכן חופשי בהודעה.
    בנוסף, כאשר נשלחת הודעה דרך האתר, אנו עשויים לראות את כתובת הדוא"ל שלך דרך שרת האתר, גם אם לא הזנת אותה באופן ישיר.
  • מידע הנאסף אוטומטית באמצעות קובצי עוגיות (Cookies) לצרכים תפעוליים, סטטיסטיים וניתוח תעבורה (Analytics).

האתר אינו מיועד לאיסוף מידע רפואי ואין להזין בו מידע רפואי אישי או רגיש.
כל טופס רפואי (כגון הצהרת בריאות) נשלח בנפרד באמצעות מערכת Jotform, שאינה מקושרת למערכת האתר.

2. מטרות השימוש במידע

המידע שנאסף משמש ל:

  • יצירת קשר עם הפונים לצורך תיאום טיפול או מענה לפנייה.
  • שיפור השירותים והתוכן באתר.
  • דיוור או עדכון פרסומי (בהתאם לחוק התקשורת – ניתן להסיר בכל עת).
  • ניתוח נתוני גלישה לצורך שיפור חוויית המשתמש.

3. מסירת מידע לצדדים שלישיים

העסק אינו מעביר מידע אישי לצדדים שלישיים, למעט אם נדרש על פי חוק או צו בית משפט.
נתוני טפסי הצהרת בריאות נשמרים באופן מאובטח במערכת Jotform בלבד, שאינה חלק מהאתר.

4. אבטחת מידע

האתר מאובטח באמצעות פרוטוקול HTTPS (SSL), המאפשר הצפנת תקשורת בין המשתמש לבין השרת.
אנו נוקטים באמצעים סבירים לאבטחת המידע, אך אין ביכולתנו להבטיח הגנה מוחלטת מפני חדירות בלתי-מורשות.

5. שמירת נתונים

נתוני הפניות נשמרים באופן מאובטח בשרתי האתר ובגיבוי פרטי לצורכי תיעוד בלבד.
המידע נשמר לפרק זמן סביר בהתאם למטרות השימוש בו, ולאחר מכן יימחק או יעבור אנונימיזציה.

6. זכויותיך לפי תיקון 13 לחוק הגנת הפרטיות

בהתאם לתיקון 13 לחוק, עומדות לך הזכויות הבאות:

  • זכות לעיין במידע שנשמר אודותיך.
  • זכות לתקן או לעדכן מידע שגוי.
  • זכות לבקש מחיקה של המידע אודותיך, בכפוף לחובות חוקיות לשמירה.

7. שימוש בעוגיות (Cookies)

managing clinical treatment centers for athletes in Tel Aviv (“the Business”, “we”, “our”).

Health declaration forms are handled separately through Jotform, which is not integrated with this Website.
